WHAT WE OFFER:
Residential Habilitation
The service provides and arranges support, training, supervision and activities to assist the person to live in their own home, Host Home or apartment.
Our service helps individuals maintain or increase their ability to perform daily living skills and increases their level of independence and integration into the community.
Provider support is based on the individual’s level of need. Supervision can range from 24 hours per day to only a few hours per week.
Day Program
Community Intergration aims to develop an individual’s awareness of their interactions and participation in their involvement the community and establish relationships with members of the community. This is achieved through exposure to and experience within the community, as well as teaching concepts such as self-determination, self-advocacy, socialization, and building of social interaction. It provides individuals with opportunities to enhance their community engagement and build meaningful connections.
Day Program services offer support and activities in group settings to help participants maintain, improve social, personal care and adaptive skills. These services are provided through a supervised environment where individuals can spend time away from home, engage in person-centered activities, achieve their goals, and enjoy new experiences.
